Ensuring Success: Managing Expectations and Treatment Adherence

Dry eye disease (DED) impacts countless individuals, and while therapies like Xiidra® (lifitegrast ophthalmic solution) 5% have proven effective,1 ensuring consistent patient compliance remains a challenge for eye care professionals. Xiidra, which can improve signs and symptoms as early as two weeks,1* is most effective when used consistently. Based on insights shared in the Defocus Media podcast, this article explores practical approaches to improving patient compliance through clear communication, setting expectations, and proactive patient engagement.

*Xiidra significantly reduced symptoms of eye dryness at 2 weeks (based on Eye Dryness Score compared to vehicle) in 2 of 4 studies, with improvements observed at 6 and 12 weeks in all 4 studies.

Understanding Compliance in Dry Eye Treatment

DED is often a chronic, life-impacting condition requiring ongoing management. Patients may not immediately see the value in treating DED, and misunderstandings can cause non-compliance. Xiidra® (lifitegrast ophthalmic solution) 5% can reduce symptoms like dryness and irritation within weeks,1* but adherence is critical to achieving these benefits.

As Dr. Jennifer Lyerly noted, explaining the purpose and potential impact of Xiidra is essential to helping patients appreciate its role in long-term ocular health. By emphasizing that regular use can yield noticeable improvements, providers can help patients see the value in their commitment to treatment.

Effective Communication in the Exam Lane

Communication is foundational to compliance. Patients need to understand not only their diagnosis but also how Xiidra fits into their overall treatment plan. As Dr. Darryl Glover shared, a well-communicated plan includes a positive, reassuring approach: “I got your back; no worries, we’re in this together.” Language like this reassures patients that their provider is invested in their success and encourages them to stick with the treatment.

Tips for Effective Communication:

  1. Use Relatable Terms: Start with familiar language, such as “Your eyes aren’t working as they should,” to ensure patients understand their condition clearly.
  2. Set Clear Expectations: Discuss potential side effects like temporary burning or an altered taste1 so patients know what to expect and aren’t surprised.
  3. Draw Comparisons to Everyday Medications: Dr. Lyerly noted that a simple analogy like comparing Xiidra to blood pressure medication helps patients understand that it only works with consistent use.

Building Compliance with Follow-Up Visits

Follow-up visits are a powerful tool for reinforcing compliance. Dr. Glover explained that scheduling a follow-up visit provides a critical opportunity to assess patient progress, troubleshoot any issues, and encourage long-term adherence. He also shared that adding a touch of humor or personalization can make follow-up instructions feel less prescriptive and more collaborative, helping patients feel at ease and engaged with their treatment.

Dr. Mark Schaefer emphasized that these visits allow both patient and provider to witness the benefits of Xiidra. Dr. Schaefer noted, “Seeing the medication work builds confidence for both the patient and my staff.” The positive reinforcement patients receive during follow-ups can significantly boost compliance rates.

Pro-Tip: For patients who may be less consistent, consider adding a quick courtesy call from a staff member between appointments to check-in. This small gesture reminds patients that their eye care team is dedicated to their success.

Addressing Side Effects Proactively

One of the main reasons patients discontinue treatment is unexpected side effects. Dr. Schaefer highlighted the importance of preparing patients for what they might experience, including mild burning, blurred vision, or a temporary altered taste.1 He explained, “Under-promise and over-deliver. Patients are pleasantly surprised if they don’t experience any side effects or find them mild.”

Dr. Lyerly echoed this sentiment, noting that patients are less likely to be alarmed when side effects are normalized. By discussing these possibilities upfront, practitioners can preemptively address concerns, reducing the risk of patients abandoning treatment early.

Integrating Xiidra into Daily Routines for Better Compliance

Connecting Xiidra use with daily habits, like brushing teeth, is an effective way to encourage adherence. Dr. Glover shared that he often advises patients to use Xiidra when brushing their teeth, making it part of a routine they already follow.

Handling Cost Concerns and Leveraging Pharmacy Partnerships

Cost can be another barrier to compliance. Dr. Schaefer and Dr. Glover discussed strategies to ease the cost burden, including partnering with specialty pharmacies or using services such as BlinkRx. These resources simplify the prescription process and help patients obtain the medication at a lower cost, reducing abandonment rates.

For patients with cost concerns, Dr. Glover suggests stating, “If it’s too much, call me. We’ll figure something out.” This reassures patients that they won’t have to navigate cost concerns alone, strengthening their trust in the provider.

Conclusion: Commitment to Patient Success

Ensuring compliance with Xiidra is a blend of patient education, empathy, and effective follow-up. When patients understand their treatment and feel supported, they’re more likely to adhere to the regimen. By setting clear expectations, building trust, and providing the resources they need, eye care professionals can help patients achieve lasting relief and healthier eyes.

Dr. Schaefer reminds us, “You’re going to workshop this and improve your approach with every patient.” Ultimately, compliance is not just about adhering to a treatment plan; it’s about fostering a trusting relationship where patients feel empowered and confident in their care.
